I've said my theory before: that their feelings aren't really a mystery to her because of her Kraang telepathy. That has got to be complicated not only for her, but for them too, if they were to begin to really grasp just how exposed their inner selves are to her all the time.
For the last year, her best friend was actually Irma, and I can believe that April being unable to sense Irma (because Kraang Subprime would never blow his cover by allowing that) probably made April feel more ordinary being around Irma—this assuming that Irma was always Kraang Subprime in disguise.
Another possibility is, April may have actually liked Irma the most, and Donnie and Casey were never serious options for her as long as she thought Irma was an option, and April felt it may have been easier to be romantically avoidant of anyone else until she knew whether something with Irma could work. Of course, even if that were the case, now that her best friend Irma turned out to be a Kraang who tried to kill her and her friends (and whom at least April believes to have always been a Kraang), it may be safe to say Irma is no longer an option for her, and now April may be relatively more open to Donnie or Casey in ways she might not have been before.
But ultimately the above may not matter if she's already long since annoyed with both Donnie and Casey's increasingly desperate and spiteful "caged animal" behavior. Donnie's overthinking and obsessive endeavors and Casey's abrasive invasive flirting may already be bad enough. But if she already knows very well how both of them feel about her and their simmering resentment of each other, then seeing them constantly try to one-up each other or tear each other apart competing over her is probably the worst thing they could be doing if either of them wants to earn her heart.
I think April was receptive to Donnie and kissed him because his moment of humble honesty was something that actually touched her in a way his previous actions never could. I mean, Donnie can be pretty arrogant, even when he's trying to be endearing. Even that music box scene was damn awkward, because Donnie overdid it by putting his photograph inside the music box to constantly stare back at April and remind her more of his obsession and desperation than anything else. But here finally was Donnie admitting that he was actually powerless, and that what April does with her heart is entirely her decision—a decision that Donnie came to believe would likely never involve him. But paradoxically, this was genuinely attractive of Donnie to do, earning April's kiss whilst simultaneously confusing the hell out of him. She may end up liking Donnie more if she can see more of this Donnie—where he's not trying so hard, not thinking so hard, not trying to put on such a tacked-on romantic façade, but level-headedly accepting his circumstances for what they actually are rather than merely what he wishes them to be. Still, if I were April, knowing Donnie, I would not expect regular miracles, and this is why I think April still hasn't made a decision.
As for whatever chance there could be that April may ultimately like Casey, that remains to be seen. Their on-screen chemistry is not nearly as well-developed as April and Donnie, who have had a lot more screen time for character development.
My issue with that is the Irma thing because if she did think she could be with her and was in love then she probably would be more upset but it was more along the lines of a surprise like "Oh my gosh I can't believe my best friend was a kraang!" Not "NOOO! WHY?!". Either that or April's just a heartless b#$ch after all she cried for like five minutes when she found out her Mom wasn't her Mom. Or when her dad got mutated again she's just like Oh well!
Oh don't worry they're just waiting for the kraang to finish transforming the citizens into ugly freaks. They'll be back once it's uninhabitable to then they'll probably just hide in the sewers cowardously for the rest of their lives.
